Please print out, complete and return this form. Make your check or money order payable to "WildAgain" and send to WildAgain at P. O. Box 685, Evergreen, CO 80437-0685. Please register early, as seminar size is limited.

The registration cost for rehabilitators or rehabilitation volunteers for the two-day seminar, including the Handbook, is $100. The cost for two rehabilitators registering from the same household (spouses or domestic partners) and sharing a Handbook is $135. (If the rehabilitator has already ordered the Handbook from www.Ewildagain.org, a discount of $30 may be indicated below reducing the cost of registration to $70 for the two-day seminar).

The seminar runs from 8:30 to 5:30 each day with sign-in at 8am. An attendance certificate will be provided for continuing education credit. A confirmation letter with maps and hotel information will be sent when paid registrations are received. Ground transportation options will also be provided from Denver's DIA airport.
