The Survey

The survey instrument is a 4-page pdf document which is opened and viewed using the Acrobat Reader software. Most personal computers (PC's or Macs) come with this pre-installed. If you do not have this free software, or need to update your version, please click on the Acrobat Reader icon at the bottom of this page to take you to Adobe Software's website for your free download.

When you download the survey document and open it in Acrobat Reader, you will be able to fill in the survey on-screen. You will respond to questions by clicking on buttons and check boxes or filling in blanks. It's easy, straightforward and takes about 20 minutes.

To submit your responses, you have several choices.

1. If your version of Acrobat Reader allows you to save the information you have typed in to the survey form, then simply save the data in the form and return the pdf file to us at wlrehabproject@aol.com.      Note! After you have filled in the form, please print a copy of the survey prior to saving the data. That way if the emailed form reaches us without any saved data, we will ask you to either mail or fax the survey to us.

2. If your version of Acrobat Reader does not allow you to save the data you have typed in, then simply print the completed form on your printer and either mail or fax the survey back to us at the address or number below.

3. Or, if you just prefer to fill surveys in by hand, feel free to print the blank survey, fill it in by hand and return to us by mail or fax.
